DOS scale

The Delirium Observation Screening Scale is a 13-item observational scale of verbal and nonverbal behaviour. The observations can be done during regular care. To optimize the recognition of delirium, the recording of observations per shift is important.

When there is a change is behaviour and you are not sure whether it is dementia or a delirium due to a new underlying disease (like cystitis or pneumonia), record 3 times a day for 3 days.
